Must have Birth Certificate for EACH child, ages Birth to 14
Proof of City of Tullahoma Residency (Ex. TUA Bill, Picture ID, etc.)
NO CHILD'S SOCIAL SECURITY CARDS ACCEPTED!!!
Need Parents/ Guardian's last 5 digits of Social Security Number
